A. The contractor will work collaboratively with the DORS Occupational Therapists (OT) to develop schematic designs of ADA and building code compliant architectural (structural, mechanical, electrical, plumbing) documents for residential home modifications that address the physical/functional capacities and limitations of the DORS consumer. B. The OT will evaluate the consumer’s physical limitations as related to home ingress/egress and maneuverability within the home. Once the OT has determined what home modifications would provide safe accessibility based on the consumer’s physical capacities and limitations the contractor will determine the home structural, mechanical, electrical and other technical requirements for feasibility of such modifications. C. After completion of the home accessibility assessment, the contractor will then write a scope of work for, and construction cost estimate of, the home modification. D. After DORS administration approves the home modification based on the OT and contractor’s schematic design and cost estimate, the contractor will prepare construction documents, including but not limited to, drawings, complete with three-part CSI specifications, and project schedules for the home modification projects. E. If land surveys are needed to create the plan drawings and specifications for outdoor vertical platform lifts (VPL) and ramps, the contractor will be responsible for ordering and obtaining those surveys. F. The contractor shall be responsible for all costs incurred in connection with the performance of the services under this contract, including but not limited to the costs of surveys, reports, permits, and any other expenses related to the execution of the scope of work. The contractor will bear full financial responsibility for these costs, unless otherwise specified in the contract. G. The construction documents consisting of specifications and drawings are used by MSDE Procurement to solicit bids, award the projects to qualified bidders, obtain building and occupancy permits from local jurisdictions and construct the projects. H. The contractor will need to attend on-site pre-bid conferences, pre-construction, construction, progress, and post construction meetings. I. The contractor will respond to inquiries concerning projects, to resolve technical difficulties and to ensure compliance with agency needs and objectives, project and design standards, contract specifications, construction specifications and drawings, applicable building codes, sound engineering practices and ADA Accessibility Guidelines. J. The contractor will provide intermittent on-site monitoring of the performance of contractors and subcontractors to assure adherence to the construction specifications, plan drawings, governmental permit standards, local and national building code standards, and ADA compliance. K. The contractor will provide a written report of what was observed, discussed and any on-site instructions, additions, subtractions and/or changes to the scope of work. The written report should also include a summary of project progress. L. The contractor will monitor work in progress, conduct inspections at project milestones. M. The contractor, in conjunction with the Contract Monitor and Procurement Officer, will approve or disapprove any change order requests, time extensions, and payment requests. The contractor must provide written justification and cost estimate for all change order requests.
