The Contractor shall provide services in accordance with Attachment 4, Service Schedule. Please note, the SMCHD Main Building, SMCHD Health Hub and the SMCHD School Based Health Centers (SBHC) contain areas where patient care is performed. Body fluids and other hazardous material may be present in these areas. The SMCHD Main building and SMCHD Annex shall have services performed daily with services being in the afternoon, via a mutually agreeable start time, and completed by 5:00 PM eastern standard time. SMCHD is requesting a Monthly rate be submitted for both locations. The SMCHD School Based Health Centers shall be completed once weekly on Fridays, starting after 3:00PM EST. SCMHD is requesting a Monthly rate be submitted for both School Based Health Centers. Please note that the School provides basic care throughout the week. The goal of these services is to supplement and ensure a fully disinfected area being provided. As such the school is responsible for replacing all supplies and removing/cleaning all trash receptacles. The SMCHD Health Hub shall be completed as needed. SMCHD will provide a minimum of 24-hour advance notice to the contractor when services are required. SMCHD is requesting a daily rate be submitted for this location.
