The Contractor shall provide plans that at a minimum, cover the following: ● Ability to pay benefits worldwide for accident and sickness (exemption from coverage in an international student’s home country is acceptable:); ● All pre-existing conditions; ● Have a maximum benefit of not less than $250,000 per incident; ● Have medical expense coverage of not less than 80% of reasonable expenses; ● Have a deductible not in excess of $100 per injury or sickness per individual; ● Accidental Death and Dismemberment; ● Repatriation of Remains of not less than $25,000 to a student’s home country; ● Medical Evacuation to a student’s home country of not less than $100,000; ● Include prescription drug coverage of no less than 50% of actual charge; ● Physician Office Visits; ● Inpatient Hospital Services; ● Hospital and Physician Outpatient Services; ● Maternity Care for a Covered Pregnancy the same as benefits for an illness (if they are not the same, the maternity benefit must meet all applicable waiver standards (e.e. deductible, maximum benefit, etc.); ● Inpatient treatment of mental and nervous disorders; ● Outpatient treatment of mental and nervous disorders; ● Treatment of drug and alcohol abuse; ● Outpatient back and spine treatment (including modalities); ● Therapeutic termination of pregnancy; ● Routine nursery care of a newborn child of a covered pregnancy; ● Annual cervical cytology screening for women 18 years of age or older;
