Specifications include, but are not limited to: The Maryland Department of Health (MDH or the Department), Prevention and Health Promotion Administration (PHPA), Maternal and Child Health Bureau (MCHB), is issuing this solicitation to coordinate the Integrated Maternal Health Services Project. This Project will consolidate and coordinate the currently disparate components of the existing perinatal care coordination systems into an integrated model that can be used across the state of Maryland. The primary objective is to improve rates at which high risk pregnant and postpartum people identified by their care team are connected to needed services in their communities. A longer term, secondary objective is to understand the impact of the Maryland Prenatal Risk Assessment (MPRA) and Postpartum Infant and Maternal Referral (PIMR) form completion on maternal health outcomes, including maternal mortality in Maryland. Achieving these outcomes will require significant stakeholder collaboration and guidance from an organization with significant quality improvement experience and comprehensive perinatal care expertise in Maryland’s outpatient and inpatient settings.
