TITLE: Federal Occupational Health (FOH) Private Provider Network (FOHNet)
I. Introductory
This is a Sources Sought. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this Sources Sought is to obtain a knowledge and information on potential vendor interest for project planning purposes.
Project Description:
The purpose of this requirement will be to acquire professional occupational health services through a nationwide Private Provider Network (PPN) to support Program Support Center (PSC), Federal Occupational Health (FOH) under the Department of Health and Human Services (HHS). The nationwide PPN include two regions, 1) FOH Private Provider Network (FOHNet) East and 2) FOHNet West. Services will be performed in the 50 United States plus; Guam, Northern Mariana Islands, American Samoa, Puerto Rico and Virgin Islands. The requirement provides integrated Occupational Health and Safety (OHS) services throughout the Federal Government.
The work performed will require the Contractor to maintain a Contractor-Owned Contractor-Operated, Private Provider Network Client Portal (COCO PPN-CP) in the designated FOHNet Region and is responsible for performing a full-range of occupational health services. The Contractor will maintain an online, secured web-base portal, also referred to as a COCO PPN-CP, to manage customer/patient service and records. The Contractor will obtain certification and maintain an HHS Authorization To Operate (ATO) certification and permanently store government/medical records.
Background:
FOH is a component of the PSC that is part of the HHS. Their mission is to provide various occupational health and related health services to Federal employees across numerous Federal agencies to promote and maintain the physical and mental fitness of the Federal workforce.
FOH is primarily composed of Service Areas that specialize in the delivery of health and health related services to customer agencies. FOH and customer Federal agencies, utilize Inter-Agency Agreement’s (IAAs) to manage business relations and coordinate the delivery of health-related services and costs associated with receiving those services. FOH has acquired occupation health support services in the Clinical regions for more than twenty (20) years. This requirement is recurring and ongoing in order for FOH to continue providing service to Federal Agencies.
Purpose and Objectives:
FOH is conducting market research to prepare for the continued requirement of providing Private Provider Network.
The purpose of this contract is to acquire professional occupational health services through a nationwide PPN to support FOH. The contractor shall manage referrals through COCO PPN-CP.
Key challenges specific to this contract the Contractor shall be aware of include:
- Management and oversight of permanent medical records (paper and electronic) to maintain confidentiality and appropriate medical record keeping.
- Maintaining communications with PPN providers regarding billing for FOH services to prevent improper billing practices or misdirected bills.
- Maintaining a PPN with providers in requested service areas that offer the services required of FOH customers.
- Scheduling and completing services within the defined timeframe(s)
- Submitting complete reports that do not contain missing items within the defined timeframe(s).
- Maintaining a PPN with personnel that are trained to meet the requirements of FOH policies and identified standards.
- Receiving all referrals through COCO PPN-CP System and/or agreed government and contractor system. FOH will place a referral into the Contractor’s secure web-based portal for action. It is the Contractor’s responsibility to check their secure web-based portal and manage each record in accordance with the timeframes outlined within the PWS. Automatically tracking and recording the date and time of many milestones from record/referral creation to submission back to FOH, is also required.
- The mobile outreach fee is utilized when the service/work order requirements clearly indicate a mobile outreach service is required.
Anticipated Period of Performance
The anticipated single award Indefinite Delivery Indefinite Quantity (IDIQ) date is First Quarter fiscal year (FY)26. The anticipated period of performance is five (5) years.
Anticipated Place of Performance
The work will be performed at the Contractor centralized location and designated Service Provision Sites (SPS), including sites that provide direct care to enable FOH to fulfill its interagency acquisitions agreements (IAAs) with other Federal agencies in the United States of America, its territories and trusts as specified in the individual task orders.

Submission Instructions
1. Interested vendors shall provide response to questions in their entirety regarding this requirement. Refer to the attached Excel copy – Federal Occupational Health (FOH) Private Provider Network (FOHNet) Questions.
2. Response Instructions
Potential vendors shall be capable of furnishing all labor, materials, and equipment required to meet the Project Requirements. Please provide supporting response to include the below information. Possible solutions and approaches that may currently exist in the marketplace, and information regarding innovative ideas or concepts.
a) Experience Information: (a) staff expertise, including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature in the past three (3) years; (d) corporate experience and management capability; and (e) examples in the last 3 years of prior completed Government contracts, references, and other related information.
Note: Please include the Customer name; Customer’s Point of Contact email address; Total Contract Value; Period of Performance; Brief description of services; Past Performance Questionnaires or Contractor Performance Assessment Reporting System (CPARS) reports if available.
b) Company’s Information: Unique Entity ID (UEI) number, Cage Number, company’s name, address, point of contact (name, email address and phone number), and size and type of business (e.g., 8(a), HUBZone, etc) pursuant to the applicable North American Industry Classification System (NAICS) code; HHS Mandatory Vehicle Use List.

Only electronic responses will be accepted. The response shall be limited to 10 double-spaced pages total. The pages will have 1-inch margins, and using not less than 12-point font, Times New Roman. Tables and figures can be Times New Roman and no smaller than 9-point font. The total number of tables and figures shall be limited to 12. The Cover Letter, Tables of Contents, and Technical Appendix are not included in the page count.
