LAL Endotoxin Tests to include: • 4 tubes, mailers, freezer packs, pre-paid mailing labels, specimen cups • Estimated 252 determinations with results faxed next day and hard copy to be mailed • If a positive result appears, then the vendor is to contact the person designated by the contract monitor immediately by the method of communication as defined by the contract monitor. 2. AAMI Water Analysis Panel Tests: • Panel #1103/2337 Iron/1242 Nickel/2353 Phosphorus, Dialysis Electrolytes & Calcium Panel #1690 • To include: 5 bottles, mailers and mailing labels. 3. Bacteria Tests (water colony count/colony count for dialysate) to include: • 4 tubes, mailers, freezer packs, pre-paid mailing labels, specimen cups • Estimated 252 determinations with results faxed the next day and hard copy to be mailed • If a positive result appears, then the vendor is to contact the person designated by the contract monitor immediately by the method of communication as defined by the contract monitor.
