The Office of Long Term Services and Supports (OLTSS) within the Maryland Department of Health (MDH) is soliciting responses from qualified providers to deliver case management services to participants of the Medical Day Care Services Waiver (MDCSW) program. Adult Medical Day Care centers (AMDC centers) offer medical day care services to individuals who have been determined eligible for the Maryland Medical Assistance Program (Maryland Medicaid), are aged 16 years or older, require a nursing facility level of care (LOC), and are choosing to receive a cost-effective, community-based alternative to institutional care. Case management services are separate and distinct from services provided by the AMDC centers, and are not delivered by MDCSW agencies or staff. Instead, case management services are provided by case management agencies (CMA), whose staff– case managers– develop initial, annual, and revised plans of service (POS) to help applicants and participants access Medicaid and non-Medicaid services. Through direct collaboration with participants and/or their representatives, POS are tailored to each participant using a person-centered planning (PCP) process. The reimbursement for case management services is an all-inclusive rate for administrative, record-keeping, and general case management tasks. The fiscal year 2025 reimbursement rate is $23.2645 per unit, with each billable unit representing a 15-minute interval for individualized case management services. The rate for case management services is established annually, or as the State budget allows, and the rate is subject to change during the term of this Agreement.
