The Community Health Worker Program, housed in the Office of Population Health Improvement (OPHI) at the Maryland Department of Health (MDH or Department), is seeking the services of a contractor to deliver virtual professional development healthcare trainings targeted to community health workers (CHWs). The instructional and supporting materials will be provided by OPHI for the trainings in the following five (5) healthcare areas: Behavioral Health, Chronic Disease: Asthma, Infectious Disease, Maternal and Child Health, and Older Adults and Aging. The successful contractor will be responsible for: 1) collaboration with OPHI on outreach to potential trainees; 2) registration for all trainings; 3) delivery of three (3) virtual trainings on each healthcare area at dates and times agreed upon by the contractor and OPHI; 5) conducting an assessment of learning by participants in each training session; 6) overall evaluation of the trainings; and 7) the completion of a final report.
