i. Develop/adopt an Interest-In-Relief Form with the ability to be submitted electronically ii. Develop/adopt an Objection form with the ability to be submitted electronically iii. Develop/adopt an award form generated electronically iv. Develop/adopt an Acceptance of Individual Relief Award and Release of Claims form with the ability to be submitted and maintained electronically v. Provide quarterly reports on actions taken vi. Assign an identification (ID) number to each of the individuals seeking relief/claimant vii. Send notices to identified individuals viii. Keep records of all Notices that are sent and or returned ix. Provide the data of all Interest-In-Relief Forms received to the Department x. Send to each claimant who submitted an Interest-In-Relief Form a Notice of Individual Relief Determination via email and first-class mail xi. Send to each claimant a Notice of Interest-In-Relief Award xii. Review the form(s) to determine whether it is fully executed with the information that is necessary to effectuate the Claimant’s Interest-In-Relief Award xiii. Provide receipt of acceptance for all forms xiv. Notify the Claimant via email, and mail, that their form(s) was not fully executed, providing a clear explanation of how his or her form was deficient xv. Provide the Department with a list of all Claimants who submitted Acceptance of Interest-In-Relief Award and Release of Claims Forms xvi. Compile the Amend Final Individual Relief Awards List
