The specific services needed for each component of the required specific services are as follows: 2.3.1 Sample Collection The Successful Bidder shall provide Sample Collection Sites meeting the following requirements: • All alcohol testing shall be performed as a breath alcohol measure utilizing accurately calibrated and maintained equipment. A reading exceeding .02 grams per 210 liters of alcohol shall constitute a violation of the applicable regulations and shall be followed immediately by a second breath alcohol test to confirm the original result. Department of Transportation (DOT) regulations prohibit the laboratory and the Medical Review Officer from being the same provider. The specimen collected shall be sent to the laboratory for processing and then reported to the designated Medical Review Officer. The quantity processed and reported shall be the same as the amount collected less alcohol. • The Successful Bidder shall provide a minimum of one (1) local, convenient federally certified facility to serve as the principal collection site where breath alcohol testing shall be performed and urine samples for CDS testing shall be collected. The facility shall meet the site requirements found in Section 2.4.1 of this IFB. • The Bidder shall state the location of the facilities to be utilized in the Bid Response. • The Successful Bidder shall provide a minimum of one (1) federally certified facility that shall provide 24/7 breath testing and sample collection services. • The Successful Bidder shall provide collections, chain of custody, and sample testing performed in strict accordance with Federal, State, and local laws, regulations, and standards. • The Successful Bidder shall provide a minimum of one (1) person who shall serve as a contact and liaison for the County and shall ensure successful achievement of the County's stated objectives. • Breath Alcohol results and chain of custodies shall be sent to the County by the Successful Bidder within twenty-four (24) hours utilizing a mutually agreed upon method. • Any incurred costs shall be invoiced by the Successful Bidder directly to the County in a consolidated monthly invoice in accordance with the IFB Specifications. • The Successful Bidder shall maintain records of all work performed for the County for a minimum of five (5) years. • Subcontractors required for the performance of this IFB shall be the responsibility of the Successful Bidder. Subcontractors shall be identified in the Bid Response with a complete description of each subcontractor's role relative to the Successful Bidder. 2.3.2 Laboratory Analysis • Successful Bidder shall review the proper chain of custody from the collection site before testing.
