Specifications include, but are not limited to: Develop and implement, within thirty (30) calendar days of the encumbrance of the resulting contract, Department-approved policy and procedures to determine eligibility for Youth. Participating Youth shall: Be between the ages of fourteen (14) and twenty-six (26); Have a diagnosis of a SED and/or SMI; and Not have insurance that covers YPSSN services. Develop and implement, within thirty (30) calendar days of the initial term of the contract, Department-approved policy and procedures for intake, including: Referral form/face sheet; Service agreement (agreement between the Youth and YPSSN); Rights of recipients; Notice of privacy practices; Consent to treat; Notification of grievance procedure; and Confidentiality agreement and release of information. ; Provide justifications for determinations and describe how community outreach will be conducted to engage Youth in System of Care activities with a specific focus on underserved, minority populations such as: Individuals with correctional involvement; and Lesbian, Gay, Bisexual, Transgendered and Queer/Questioning Intersex and Allied individuals (LGBTQIA). Ensure YPSSN services include sustainable means of support and communication and education groups and/or Drop-in Opportunities and describe how the Youths’ feedback from the Youth Satisfaction Survey and other communication with Youth will be used for future implementation and development of YPSSN. Utilize technology and social media to meet the goals of engaging Youth, promoting the YPSSN, and promoting Resiliency and Recovery supports.
