Specifications include, but are not limited to: Task 1: Lead Project Oversight (280) Contractor shall attend the Lewiston Lead Program (LLP), project contract signing, and oversee the construction phase to assure compliance with state and federal guidelines. The LLP shall provide a copy of the Lead Hazard Control (LHC) Design, and copies of the Lead Inspection-Risk Assessments for each unit. The Contractor shall coordinate with the abatement contractor to understand the plans for project containments. Contractor shall collect and submit copies of the required documentation from the abatement contractor (DEP Notifications, Occupant Protection Plans, and any building permits required) to the LLP. The Program must be notified of the work start date in advance so the required Section 3 sign is secured in place at the project site. Contractor shall perform a relocation needs assessment for each unit and verify tenants are properly notified, instruct occupants on how to prepare personal belongings for the LHC activities, obtain proper sign-offs on the relocation agreement, and submit it to the LLP for processing. Contractor shall deliver relocation per diem checks to tenants at relocation time and obtain a receipt sign-off. For tenants that choose to self-relocate, Contractor shall confirm unit relocation start date, verify that the unit is prepared for containment, release the per diem payment, and contact the tenant when the unit is safe to return. A portion of the tenants will require program provided relocation (a hotel room). The Contractor shall book the room dates needed during the LHC work phase and cover the lodging costs. Accommodations must be reasonably priced and located in the area. Kitchenette amenities, and family transportation for work and school needs should be considered when choosing the hotel. Room charged incidentals, phone calls, internet, and any smoking or specialty cleaning fees are the responsibility of the tenant. Hotel lodging is a separate reimbursement cost paid by the Lead Program. Lodging reimbursement requires receipts and may be billed after the unit has passed clearance and relocation has been completed. Contractor shall collect invoices from the abatement contractor for billable activities completed. Contractor shall inspect and verify the activities or materials on site billed meet the design requirements. Contractor shall obtain necessary sign-offs for the Payment Release and submit with the acceptable invoices to the LLP. Once a check is cut, Contractor shall distribute the payment to the abatement contractor, obtain a signature of receipt and a signed Release of Lien. Contractor shall return copies of the Receipt and Release of Lien to the LLP. Contractor shall conduct and document site monitoring visits to assure safe work practices, conduct paint scrape inspections, and collect site photographs during the project LHC activities. Contractor shall prepare change orders when required, with written specifications, photographs, any hazard determination documents needed, and obtain necessary sign-offs. Contractor shall conduct Clearance Inspections and dust/soil sampling upon notification from the abatement contractor. Clearance Inspections, Lab sample submissions, and Lab results reported back must be handled efficiently for possible re-cleaning and containment break down, keeping relocation times minimal. Current HUD program dust action levels are lower than Maine DEP action levels. The most stringent dust levels apply to LLP activities. Lab testing fees are a separate reimbursement cost paid by the Lead Program. Contractor shall obtain Code Enforcement sign-off and conduct a final inspection of all LHC areas with the Owner and Program Manager. Contractor will document a punch list and verify completion before the last payment is released. All required documentation may be submitted electronically (preferred) or delivered in paper copy. Project closeout packet will include site visit documentation, project photos, Letter of Hazard Control, and a copy of the Abatement Contractor Clearance Report (which should have DEP Clearance Form M(s) and the Lab testing results). Closeout packet shall be provided to the Program at project completion (for 280 units).
