Specifications include, but are not limited to: Link families to information and support to assist in improving health outcomes for their Children with Special Health Care Needs by:
Promoting the Family Navigation services through outreach; and/or
Ensuring the awarded Bidder’s contact information is updated and accurate within the 211 Maine system.
Respond to requests for assistance within forty-eight (48) hours of receiving the request.
Link families to Family Navigators and other Community-Based Supports and Services, as needed.
Maintain up-to-date information on Community-Based Supports and Services.
Assist families in navigating the health care system, which may include:
Providing guidance through the completion of eligibility applications;
Providing supporting in preparation of service meetings;
Providing individual assistance with assistive technology; or
Locating special medical services.
