Specifications include, but are not limited to: Conduct Assessments of the communication abilities of Clients with an Intellectual Disability (ID) and/or Autism Spectrum Disorder (ASD) who are diagnosed as deaf, hearing impaired, or otherwise experiencing communication difficulties because of expressive and receptive communication challenges when referred by the Department. Completed assessments within three (3) months of the date of referral. Maintain a record of all referred and assessed Clients to include at a minimum: Individual’s name; Unique identifying information; Referral date; Assessment date; and Disposition/referral information. Conduct assessments in-person unless the Client requests another preferred environment (e.g., virtual assessment if technology permits). Examine and evaluate service activities and goals of the Client’s Person-Centered Service Plan (PCSP) to ensure their communication needs are adequately considered. Offer suggestions to the Person-Centered Service Plan (PCSP) planning teams for consideration or inclusion in the Client’s PCSP, as appropriate. Deliver case specific consultation services to support staff and family members of the referred Client, as appropriate. Facilitate referrals to qualified service providers of Clients who need non-traditional communication services to qualified service providers.
