Assessment and Evaluation Conduct in-person Psychological-Educational Evaluations of student-residents at the AR Gould School/LCYDC., specifically and as defined in the Definitions: Autism Diagnostic Observation Schedule (ADOS); Autism Diagnostic Interview–Revised (ADIR); Social Communication Questionnaire (SCQ); General Anxiety Disorder 2-item (GADS2); Activities-Specific Balance Confidence Scale (ABC); Autism Screen Instrument for Education Planning (ASIEP3); Childhood Autism Rating Scale (CARS2); Quantitative Checklist for Autism in Toddlers (QCHAT); Autism Spectrum Quotient (AQ10); Wechsler Scales; Stanford Binet Intelligence Scales (SB-5); Woodcock Johnson Tests of Achievement; Kaufman Test of Educational Achievement, Third Edition (KTEA-3); Wide Range Achievement Test (WRAT5); Peabody Individual Achievement Test-Revised (PIATR); KeyMath3 Diagnostic Assessment (KeyMath-3 DA); and Test of Written Language-Fourth Edition (TOWL4). The nature of these assessments is to identify and/or confirm the following diagnoses as recognized in the DSM-V and supported by IDEA: Autism Spectrum Disorder (ASD); Specific Learning Disability; Intellectual Disability; and Emotional Disturbance. Communicate results, findings, and recommendations to the AR Gould special education department Provide families and AR Gould School/LCYDC with completed Psychological-Educational Evaluations. Required Reporting Provide a monthly itemized claim to accompany billing invoice, template to be supplied by the Department upon executed Contract. This document shall include, but not limited to: Demographic information; Relevant dates of service; Evaluation information (date, who completed evaluation, etc.); and Evaluation results/outcomes. Additional Requirements Ability to attend the Long Creek Youth Development Center prescribed contractor training prior to implementation of services and/or facilitating independently. Ability to conduct background checks on all employees, temporary staff persons, persons contracted or hired, consultants, volunteers, students, and other persons who may provide services under this Agreement. Confirmation of the pass/fail results of each background check upon request. The cost of performing each background check shall be the responsibility of the Provider. The methods of performing the background checks must include information from the Bureau of Motor Vehicles, the Sex Offender Registry, and the Maine State Bureau of Investigation. If services to be provided under this agreement include services to minor children, then the background check will include information from the Department’s Office of Child and Family Services regarding substantiated findings of abuse or neglect of a child. If services to be provided under this agreement are to be performed by a person who is professionally licensed, then the background check will include information from the appropriate licensing board or entity regarding the status of the person’s license.
