Louisiana State University and A&M College desires to enter into a term contract for disposal of infectious waste materials as defined by the Louisiana Department of Health and Hospitals in accordance with the specifications hereinafter outlined. Pickup locations may include the Student Health Center, School of Veterinary Medicine, Environmental Health & Safety, Ag Center Reproductive Biology Center, Pennington Biomedical Research Center and Athletics. Separate purchase orders may be issued for each location. Bids Requirements: • Bids are to be based on furnishing labor, containers, packaging materials and equipment necessary for pick up and disposal of waste under Department of Transportation & Environmental Protection Agency regulations. • All items are to be incinerated at a Medical Waste Incinerator permitted for burning infectious waste. • Supplier shall take title to the waste at the time the waste is fully loaded onto the supplier’s truck and thereby agrees to take responsibility for loading, transportation, treatment, and final disposal. • Pickups will be weekly unless notified otherwise by the using department. The University reserves the right to change the schedule in order to meet the needs of the quantity produced. • When called for an emergency pickup, a Supplier must be able to be onsite as soon as practicable for a waste pickup after the call for an emergency pickup. • During a declared emergency or during hurricane recovery efforts, the University reserves the right to add pick up locations and add multiple pickups per day per location should the need arise. • No specific volume can be guaranteed. • Supplier may be asked to provide in-service education to the LSU staff handling infectious waste at each location. • A copy of the manifest is to be left at each location upon removal of the waste. After incineration, supplier is to provide final manifest documentation to LSU that ensures proper disposal was performed. • Supplier is to provide each location with regulatory compliant fiber boxes approximately 15.5” x 17.5” x 21” with regulatory compliant inner bags/liners. • Fiber boxes, bags/liners must be included in the cost of the service and shall be provided by the supplier for each pickup location . • Transportation pricing must be included in price for disposal. Qualifications - Bids should include a resume of the Supplier’s expertise, experience and ability in the field of medical/infectious waste disposal service. Resume should include transporter EPA ID numbers and Incinerator permit information. A list of clients should also be included. Resume and list of clients must be submitted upon request within the timeframe specified by LSU in the request.
