Specifications include, but are not limited to: Rehabilitation Services - Speech, OT and Physical Therapy. Serve the resident population regardless of race, creed, color, sex, or national origin. The resident population of the facility consists of U.S. military veterans. Provide appropriate evaluations, development of plans of care, implementation of rehabilitation requirements of such care plans, and evaluation of care and treatment under authorization of specific physician orders. Services shall comply with established professional standards of practice. Documentation of all services provided shall be made in, and will become a part of the facility’s resident medical record, and of a format, style and quality established by the facility. The facility utilizes Point Click Care software. The contractor’s documentation system will be required to fully integrate with Point Click Care and must insure facility compliance with Medicare, Medicaid and Veterans Administration regulations. Quarterly, conduct in-service training to medical, clinical, and nursing staff to enhance their knowledge, skills and abilities to include but not limited to, body mechanics, positioning, range of motion strength, ambulation, operation of equipment, Medicare & Medicaid requirements as they relate to therapy etc., in order to ensure continuity, optimum patient care and smooth transition from rehab therapy to facility services. In conjunction with facility staff, develop an acceptable timeframe for the completion of initial evaluations in order to comply with requirements of Medicare/Medicaid and Veterans Administration guidelines and regulations. Provide timely and complete rehabilitative care planning as evidenced by the presence of written goals, objectives, and interventions in compliance with Medicare, Medicaid, Veterans Administration and facility guidelines, regulations and policies. Provide treatments as scheduled and/or required in compliance with Medicare, Medicaid, Veterans Administration and facility guidelines, regulations and policies. Attend/Participate in facility administrative and clinical meetings and conferences and provide input and consultation as needed and requested by the facility and in compliance with Medicare, Medicaid, Veterans Administration and facility guidelines, regulations and policies. These meetings may include but not limited to: Admission meeting(s), daily morning/standup meeting, department leader meeting(s), Medicare meeting(s), care plan meeting(s), QAPI meeting(s), safety meeting(s), etc. Submit a monthly report that shall include names of patients treated, dates of service, type of service, medical record number and unit. Attend schedule billing or triple check meetings. The format of the monthly report will be mutually agreed upon by Contractor and Facility. Provide a management plan which describes administration, management, and key personnel.
