Specifications include, but are not limited to: 1. Performs in-depth nutritional assessments upon admission, quarterly, annually, and when a significant change occurs for the residents of Carl M. Brashear Radcliff Veterans Center. Maintains accurate documentation and tracking of resident’s medical nutritional therapy and nutritional risk e.g. weight loss, thickened liquids, wounds, feeding tubes. 2. Documents nutritional care plans and makes written evaluations, maintains a specific care plan for each resident and updating to reflect ongoing changes of residents status. Plans and supervises the implementation or therapeutic diets and writes orders as prescribed by physician. 3. Consults with other health care professionals for residents’ medical nutritional therapy.
