This Request for Proposal (RFP) is being offered to establish a Personal service contract for qualified MDS Coordinator services, to individuals or entities for the Carl M. Brashear Radcliff Veterans Center located in Radcliff, Kentucky. To provide qualified Individuals and entities and auditing services related to regulatory compliance, MDS’s and care planning, billing and reimbursement accuracy and as described herein for the for KDVA and OKVC and its Veterans Centers: Carl M. Brashear Radcliff Veterans Center (CMBRVC – 120 Beds). 2.00 Scope of Work The duties and services required of Vendor shall include but are not limited to as follows: Provide the licensed staff (RN) services by licensed personnel as authorized by the Commonwealth of Kentucky or recognized reciprocal state board to the RVC. Ensure that staff continue to maintain current credentials while providing services to the RVC. Provide to two (2) work references on candidate, copy of drug test, copy of background check (National Background check with fingerprinting, KARES is preferred). Provide a copy of tuberculosis test (must be a two-step completed in the past year) and send copies of the employees annual TB testing every year for the duration that the person works at the RVC. Maintain a current listing of Clinical Staff, if the same staff person is not utilized, in a pool available for the RVC location to be reviewed by RVC Administrator or designee if requested. Follow confidentiality protocol and ensure execution of these protocols with their staff.
