FY19 Bingham Garden Administrative Management Fee/PSC-Management Consulting 2.FY19 Bingham Garden Operations Reimbursement This Request for Proposal (RFP) is being issued for the purpose of establishing a Personal Service Contract with a qualified organization to provide management services for six (6) four-bed intermediate care facility (ICF) homes located in Louisville, Kentucky. Each of the six (6) homes is 3,500 square feet. Four (4) homes are designated for general use and two homes are designated for intensive use for individuals with more severe disabilities. The Bingham Gardens ICF Homes are located at 4401, 4403, 4405, 4407, 4409 and 4505 Louise Underwood Way, Louisville, Kentucky. The homes are designed for four residents in each home with each having a single bedroom and bath and shared kitchen and common area. The facility had an average daily census of twenty-four (24) for state fiscal year 2017 and an average daily census of twenty-one (21) for fiscal year 2018. On average, there are two admissions to Bingham Gardens annually. Patient demographics for current residents are included in Attachment B.
