Perform psychological and psychiatric examinations and evaluations as needed, based on referrals and consultations with facility medical provider, administrator, social services, and medical staff with approval by the Medical Director. Complete evaluations, which include a separated diagnostic section, legibly and in a timely manner consistent with standards of care. Provide medication evaluation and written recommendations for prescribed medication to the facility medical provider. Document that informed consents have been obtained from the Veterans/responsible parties. Establishing treatment plans and participate as a member with the Interdisciplinary team (IDT) to develop care plans and be available for care plan meetings when requested.
