RANDOM DRUG AND ALCOHOL SCREENING The Robert E. Spiller Bowling Green Veterans Center (“BGVC”), is issuing this Request for Proposal (“RFP”) to provide drug and alcohol testing services to employees, contractors, and potential employees of the BGVC. 1. Establish and maintain a comprehensive network of testing sites to ensure applicants seeking employment in a test designated position at the BGVC and a test designated employee employed at BGVC who provide support services to a Veteran/family/visitors are to report when a drug or alcohol test is required for employment or continued employment. Collection site must be able to collect urine sample and breathalyzer to be forwarded to the contractor’s lab for testing. Contractor at collection sites shall have their staff require all persons required to provide a urine specimen for testing present a picture ID before a sample is collected. 2. Main testing site must be within twenty (20) miles of facility with daytime Monday through Friday hours. Must have after-hours and weekend site(s) available within thirty (30)-mile radius that can provide the same service as the primary site. Additional collection sites may be added during the term of the contract. 3. Will provide BGVC with a list of names selected from active employee register for random drug testing. The random drug test list will need to be sent monthly. 4. Provide chain-of-custody system that ensures all samples are adequately safeguarded against loss, substitution, and claims of false or broken chain of custody.
