4.1.1. 1 st layer PAPER/FORM: • 28#/pound MOCR (Made for Laser Printing) 8.5”W x 11”H loose-leaf bright white acid free paper that shall work in laser printers. Thickness of an average of 5 mil. • Pre-printed/pre-numbered control number located in upper right-hand corner of paper/form; allow 0.75” margins from top and right edge of paper/form. • Control number font style and size: MyriadPro-Regular, 12 pt; control number to be consecutive numbering [eight (8) characters] in ink color Pantone 289. Note: For this contract, the section below the clear cover/patch and label on the paper/form will remain blank. However, this paper/form with clear cover/patch and label applied shall allow for error-free laser printing for end user. View Attachment B below– This demonstrates what the end user will be laser printing onto this paper/form with clear cover/patch and label. It’s essential that the finished product of this RFP shall allow ease-of-use without errors, (i.e. no paper jams, no smudged ink, etc.). Final end product shall appear professional. 4.1.2. 2 nd layer CLEAR COVER/PATCH: • One clear cover/patch per paper/form. • 2.2 mil thick 22# Natural Kraft paper (silicone spot is placed on this at press) • View Attachment C below – This demonstrates how the clear cover/patch will appear on paper/form and placement. • View Attachment D below – Demonstrates how the clear cover/patch and label will appear on form in precise measurement placement detail.
