Specifications include, but are not limited to: 4.2.1 Contractor shall perform preventive maintenance on the lift equipment identified in Attachment D at the times that meet or exceed the manufacturer recommendation, but not to exceed three (3) times per year per piece of equipment. 4.2.2 Contractor shall provide the Fleet Services Manager or designee a signed copy of the preventive maintenance services performed within 72 hours of service date. This form shall include date of service, location, equipment type, manufacturer/model number, serial number and City equipment number. 4.2.3 Contractor shall also perform repair services on equipment, upon written approval by the Fleet Services Manager or designee, for items discovered during preventive maintenance services. 4.2.4 Contractor shall provide a not-to-exceed price for the repairs and include a detailed breakdown of the cost, including estimated hours of work, job classification, hourly rate, detailed equipment list, equipment costs, detailed material list, detailed material costs, delivery schedules, etc. 4.2.5 Replacement value shall also be provided on the work order form in order for the City to determine if it would be more cost effective to replace rather than repair the equipment. The City reserves the right to seek bids from additional vendors for any estimate with a total exceeding $5,000 due to repairs required beyond the pricing established.
