Specifications include, but are not limited to: 1.3.1.1 Brief summary of professional qualifications of Iowa licensed physicians or medical practitioners providing Optometry Services under the agreement for the records of Agency. 1.3.1.2 A photocopy of a current Iowa license for all medical providers providing Optometry services under the agreement to Agency. 1.3.1.3 On campus services to include, but not be limited to, providing routine/basic Optometric procedures including complete annual exams, fluorescein dye, IOP check, acute exams (infection, corneal abrasion, sty, scratches, etc.), cataract screening, and surgical follow up, and adjustments through consultation with members of Agency medical staff. 1.3.1.4 Written reports following the patient visits. 1.3.1.5 Frequency of visits to be determined entirely by the needs of Agency and at request and direction of the members of Agency medical staff. The Agency will require a minimum of 70 residents per month, not to exceed 85 residents per month and not more than five (5) clinics per month. IVH medical staff will refer resident/patients to the contractor. IVH will provide on-site office space for the convenience and safety of our resident patients. 1.3.1.6 Will deliver services that meet the standards for quality of service by the licensing agency for these services. Must be skilled working with elderly, chronically ill, and cognitively and physically challenged patients.
