A. Accept all Reentry Aftercare youth referrals for the Fifth Judicial District JCS for youth who are ordered to a QRTP, The State Training School or another JCS court ordered residential setting and will: a. Make contact with the Reentry Aftercare youth and their family/caregivers within 10 days of the referral from JCS and; b. Establish connections and maintain regular contact with JCS staff to facilitate the referral process. B. Respondent will establish connections with youth and their families/caregivers including but not limited to: a. Assigning dedicated staff to this project, meaning fulltime personnel whose sole duty is providing Reentry Aftercare Services for the Fifth Judicial District JCS. b. Providing options for meeting with youth and their families/caregivers based on the needs and capacity of the youth and families being served, such as contact through face-to-face meetings, video-conferencing and/or phone calls. c. Establishing positive and collaborative relationships with QRTPs, the State Training School, and other residential facilities where JCS youth from the Fifth Judicial District JCS are placed to ensure effective Reentry planning.
