1. Provide evidence of successful implementation of school-based mental health programs, including staffing credentials, licensure, and relevant experience in K-12 educational settings. Include data on program outcomes, effectiveness, or impact when available. 2. Indicate the number of hours and days staff will be assigned to each school building. While the School prefers continuity of care through having one dedicated provider assigned to each building, we recognize that daily onsite coverage may not be feasible. Proposals should clearly outline the anticipated frequency of onsite services (e.g. weekly, bi-weekly), any plans for rotating or floating coverage, and how telehealth or hybrid models may be leveraged to maximize student access and reduce wait times. 3. Clearly identify any materials, equipment, facilities, technology, or school personnel that will be required to support the delivery of services under this proposal. 4. Describe all levels of services offered (universal, targeted, and intensive). Include evidencebased interventions, group and individual supports, crisis response protocols, and how the services align with the School’s multi-tiered system of support, mental health protocols, and trauma-informed care frameworks. 5. The contracted agency will be responsible for maintaining adequate insurance coverage for all personnel providing services under this agreement. Respondents must submit proof of current professional liability and general liability insurance for the agency. The agency will ensure that all personnel delivering services under this contract are appropriately licensed and covered underthe agency’s insurance policies. 6. Agencies will be responsible for handling all applicable billing and reimbursement through Medicaid, private insurance, or other third-party payers. The School will not be responsible for any direct billing, collection, or reimbursement activities related to individual student services. Any proposed pricing should reflect only costs directly billed to the district for contracted services beyond what is covered through third-party payers. 7. Describe any additional services offered beyond direct clinical care, such as staff professional development, family education, parent engagement, student wellness programming, school climate initiatives, or staff wellness supports. 8. Describe the format, content, and frequency of reports that will be provided to the School. Reports should include utilization data, de-identified outcome measures, referral status updates, and compliance documentation.
