1. Provide evidence of successful implementation of school Health and Student Behavior programs, including staffing credentials, licensure, and relevant experience in K-12 educational settings. Include data on program outcomes, effectiveness, or impact when available. 2. Indicate the number of hours and days staff will be assigned to each school building. While the School prefers continuity of care through having one dedicated provider assigned to each building, we recognize that daily onsite coverage may not be feasible. Proposals should clearly outline the anticipated frequency of onsite services (e.g. weekly, hours), any plans for rotating or floating coverage. 3. Identify any materials, equipment, facilities, personnel, and technology that will be required to support the delivery of services under this proposal. The materials, supplies, personnel, technology, tools and equipment will be the responsibility of the contracted agency. The school will be responsible for providing a room for occupation. 4. The contracted agency will be responsible for maintaining adequate insurance coverage for all personnel providing services under this agreement. Respondents must submit proof of current professional liability and general liability insurance for the agency. The agency will ensure that all personnel delivering services under this contract are appropriately licensed and covered under the agency’s insurance policies. 5. Agencies will be responsible for handling all applicable billing and reimbursement through Medicaid, private insurance, or other third-party payers. The School will not be responsible for any direct billing, collection, or reimbursement activities related to individual student services. 6. Describe any additional services offered beyond student health and student behavioral health services. 7. Describe the format, content, and frequency of reports that will be provided to the School. Reports should include utilization data, de-identified outcome measures, referral status updates, and compliance documentation. 8. School employees and dependents enrolled in School’s group health plan will not be billed for using and will not be eligible to use the student health clinic.
