Component A titled Quality Assurance System involves the following: 1) A reliable incident management and investigation processes; 2) Protocols that ensure compliance with reporting, reviewing, and response requirements; 3) Effective mortality reviews of unexpected deaths; and 4) Mechanisms that ensure the delivery and fiscal integrity of appropriate home and community-based services. Requirements include: (1)The Contractor must have experience in quality assurance and quality improvement processes; (2) The Contractor must have knowledge of individuals with I/DD, TBI, other disabilities, and the aging population in Indiana: (3) The Contractor must possess experience in HCBS waivers overseen by the Centers for Medicare & Medicaid Services (CMS); and (4) The Contractor must be certified as a Quality Improvement Organization (QIO) or QIO-like organization by CMS. Component B titled Surveys involves the Contractor having subject matter expertise in the facilitation, data collection, analysis, and dissemination of information as it relates to National Core Indicators (NCI) surveys, Case Management Organization Satisfaction Surveys, and Consumer Assessment of Healthcare Providers and Systems (CAHPS) Surveys. Requirements include: (1) The Contractor must have experience in these types of surveys; (2) The Contractor must have knowledge of individuals with I/DD, TBI, other disabilities, and the aging population in Indiana; (3) The Contractor must possess experience in Home and Community-Based Services (HCBS) waivers overseen by CMS; and (4) The Contractor must be certified as a QIO or a QIO-like organization by CMS. Component C titled Independent Assessments of 1915(b)(4) involves conducting independent assessments of the implementation of the 1915(b)(4) waiver for Community Integration and Habilitation (CIH), Family Supports (FS), Money Follows the Person-Community Integration and Habilitation (MFP-CIH), Health and Wellness (H&W), Traumatic Brain Injury (TBI), Money Follows the Person-Health and Wellness (MFP-H&W), PathWays Fee-for-Service (PW-FFS), and Money Follows the Person-PathWays (MFP-PW) using data available from the Bureau of Disabilities Services’ (BDS) systems, as well as data from previously completed interviews and surveys. The Contractor must be certified as a QIO or a QIO-like organization by CMS.
