Specifications include, but are not limited to: A qualified individual must assess a child placed in a QRTP within 30 days of the start of each QRTP placement. The qualified individual may conduct this assessment prior to the placement in the QRTP, but must complete it no later than the end of the 30 day period. ; The assessment tool used by qualified individual to conduct the 30 Day Assessment must be age-appropriate, evidence based, validated, functional, and approved by the ACF ; • Assessments are completed within a 30 day window of a child’s placement in a QRTP (either 30 days prior to placement or within 30 days after placement) • Assessments are completed for eligible children referred from DCS or the Indiana Juvenile Justice System (JJS) • Assessments are completed by a “qualified individual” • Assessments are completed in conjunction with the child’s family and permanency team • Assessments are completed using an ACF approved assessment tool • Assessments create specific and unique recommendations for a child’s care beyond assessing QRTP placement.
