Specifications include, but are not limited to: 1.Present a service model to assist IPTC in controlling health care costs; that currently aligns with the wellness program for the IPTC workforce, including the criteria for employees outlined in the collective bargaining agreement between IPTC and the ATU, which is as follows:a.Annually, complete one or more health screenings.b.Annually, complete one or more health assessments.c.Annually, complete four (4) or more counseling sessions with a health coach.d.Annually, complete one or more other health activities sponsored by IndyGo or the wellness provider.2.Present a service model that minimally meets the current service level model; more importantly maintains a superior performance and quality assurance standard while having a high trust/privacy component.3.Prospective provider shall provide a thorough analysis of plan participant and their spouse/dependent(s)claims history (medical and prescription through carriers) and assist Client and consultant in evaluating opportunities for employee engagement and behavior modification. Prospective provider will collaborate with Client’s consultant with regard to the consultant’s population health data platform and utilize data within the platform.4.Provide a thorough overview of the vision of tying together claims activity, biometric and health risk assessment data; utilizing this information to provide a very thorough and comprehensive plan of action in meeting the unique health challenges of IPTC’s population. This approach should include a communication plan to IPTC team members that will allow for measuring the financial impact on the health plan through claim costs reduction. Prospective provider will collaborate with Client’s consultant with regard to the consultant’s population health data platform and utilize data within the platform. 5.Maintain HIPPA/PHI compliant electronic record keeping of the clinic claims and be able to share that data with plan participants’ personal physician.
