Specifications include, but are not limited to: provide comprehensive vision care benefits to Lake County employees, dependents, retirees and COBRA participants; A. The effective date of the contract will be January 1, 2021. B. All contract or benefit changes will only occur on January 1 unless previously agreed to in writing by all parties. C. Vision care benefits will be offered to all benefit eligible employees, their dependents, retirees, and COBRA participants. D. The program is 100% employee paid and is 100% voluntary benefit. E. There is a three tier rate benefit coverage structure: 1. Single 2. Single plus one 3. Single plus family F. The Proposer shall provide both in-networks as well as out-of-network benefits. The network of providers should be comprehensive, including major retail chains and independent eye doctors. G. The Proposer shall have sufficient ophthalmologists and optometrists located throughout Lake County, IL and SE Wisconsin. H. All providers shall be licensed to practice within the State of IL and preferably the State of WI. I. The provider directory shall be current and indicate which providers are accepting new patients, and which are closed to new patients, if any. The directory should also indicate which providers are chain/retail versus independent offices.
