Specifications include, but are not limited to: Drug testing services, including screening and examination, will be required 7 days a week, 24 hours a day. It is estimated that an average of 5,000 people will be tested, screened and examined. The awardee will be expected to provide services at CPD locations as well as at their own facility. If CPD employees opt to go to the awardee’s facilities, the awardee must perform all required services during their normal operating hours. 12-Panel Drug Testing Services The collection and testing of one or all of the following samples: (i) urine; (ii) blood; (iii) hair; and/or (iv) oral fluid. Vision Test The Awardee will conduct the standard vision/eye test using the Snellen Chart. Hepatitis B Shots Test will be administered by a RN, certified medical technologist, or PH phlebotomist. The test requires intravenous blood drawing with a sterilized hypodermic needle. Physical Exam and Treatment Services The Awardee will conduct a physical exam of the employee and diagnose any injury or illness. Additionally, the Awardee will provide medical treatment to the employee until proper medical clearance has been received. The exams will also include x-rays of the employee when applicable. The Awardee will issue an opinion report on whether the employee is capable of returning to normal duties required within their current position and outline required restrictions if needed. Pre-employment a nurse or medical doctor will: 1. Review an applicant’s prepared medical questionnaires and conduct a comprehensive physical exam to ascertain whether an applicant can perform the job duties with or without accommodation. 2. Will conduct exams accompanied by physical agility and functional capacity evaluations to ensure candidates are medically and physically capable of performing the job duties for which they are being hired. Supplies and Equipment The Awardee will provide all necessary consumable supplies, materials, equipment, office locations, and mobile equipment to perform the needed services. Such materials, equipment and consumable supplies include but are not limited to: sealed collection containers and tamper proof evidence tape, specimen boxes and envelopes, blood drawing devices and all other forms as required for drug testing. Inspections All services will be subject to inspection by the CPD. Services that do not comply with the specifications will be revised by the proposer if determined unfit or unsuitable by the Chicago Park District at no additional charge to the Chicago Park District.
