Specifications include, but are not limited to: The Vendor shall provide all labor, materials, tools and equipment required to remove and replace six (6) existing storm sewer inlet structures. Area #1 - 24” inlet replacement east side of south roadway old Forensic Treatment Program (FTP) Building. Saw cut and removal of curb and paving to allow for replacement. Install new 24” precast inlet. Backfill with stone aggregate. Replace curb and provide concrete road patch. Area #2 - 24” inlet replacement west side of roadway old FTP Building as follows: Saw cut and removal of curb and paving to allow for replacement. Install new 24” precast inlet. Backfill with stone aggregate. Replace curb and provide concrete road patch. Area #3 - Stores Building center of south roadway as follows: Investigate collapsing structure. Determine whether structure can be abandoned or needs to be replaced. If structure is still active part of storm system, remove old structure and replace. Patch approximately 10’ by 10’ area of pavement around structure with new concrete. Area #4 and 5 - Stores and Dietary loading dock ramps as follows: Remove concrete loading dock ramps and trench drain to facilitate replacement of structure. Install new precast concrete structure. Replace trench drain sections that need to be removed. Replace concrete loading dock ramp. Area #6 - Loading dock cone west of Stores Building loading dock entrance as follows: Replace cone on existing structure west of loading dock. Provide 10’ by 10’ concrete patch around structure. The Vendor shall be responsible for any soil testing required by the Illinois Environmental Protection Agency and submission of Clean Construction or Demolition Debris (CCDD) Forms if necessary (https://www2.illinois.gov/epa/topics/forms/land-forms/Pages/ccdd.aspx ). The Vendor shall be responsible for the removal and disposal of all waste generated during these services. All waste shall be disposed in accordance with all standards or codes issued by EPA in accordance to waste and its disposal. All work is to be completed during regular business hours of 8:00am to 4:00pm, Monday through Friday, unless otherwise approved by the Contract Monitor.
