Specifications include, but are not limited to: It is the intent of the Health Department to secure services from interested Contractors to provide Population Health Management and registry software to support the following general functions between the Health Department and approximately five (5) local hospitals: Clinical decision support to compare, remind and guide clinicians. Task and Case Management Disease and Population Management Performance feedback. All Contractors must: a. Ensure the support will meet the needs of the Health Department administrators and staff. b. Demonstrate financial stability. c. Disclose any third-party supplier relationships, including strategic partnerships. d. Have the ability to recommend and provide a variety of support models including a comprehensive implementation and transition plan.
