1. Description that Pharmaceutical Provider will meet, or exceed all specifications listed herein and be qualified, licensed, and capable of providing approved drugs, pharmaceutical supplies, IV solutions, and biological items as required by the residents of Valley Hi Nursing Home and approved by appropriate physician orders. 2. Bidder as “Contractor” will supply approved drugs, IV solutions, biological, and supplies for the Valley Hi nursing home. Proposal shall specifically be in compliance with generally accepted professional principles, any, and all applicable laws, ordinances, rules, or regulations of the State of Illinois, the United States, the County of McHenry or any other applicable regulatory, or governmental body. 3. Proposal must clearly and accurately describe the responsibilities for which the bidder, and Valley Hi Nursing Home, will be responsible for, respectively. 4. Proposal shall include the fee schedule, services provided, and Contractor responsibilities for the period of operation as outlined in this document. 5. Proposal shall include a list of the top 25 drugs commonly furnished to nursing homes, typical quantity furnished, type of packaging, and list price and current proposed price of each item by resident pay source type. 6. Bidder as “Contractor” shall provide Pharmacy Consulting Services; duties include: o General supervision of all Valley Hi pharmaceutical services and procedures for the control and accountability of all drugs, IV solutions, biologicals, and supplies o Confirm compliance of policies and procedures with applicable laws and regulations o Review records of receipt and disposition of controlled drugs o Review the resident drug regimen and report any irregularities to the Valley Hi Administrator, Assistant Administrator, Director of Nursing, and/or Assistant Director of Nursing o Consult and make recommendations with the attending physician as required o Inspect nursing stations, medication rooms, drug storage areas, medication carts, and resident health records and document findings for the consultant’s report o Ensure that proper labeling of all pharmaceuticals is within required standards o Provide written reports concerning the status of Valley Hi’s pharmaceutical service and staff performance to their Administrator, Assistant Administrator, Director of Nursing, and Assistant Director of Nursing o Participate in monthly scheduled Quality Assurance Committee meetings, Antibiotic Stewardship Committee, Behavior Modification Committee, and any other necessary meetings as warranted o Develop and/or conduct 2 in-service programs per year o Assist in developing policies and procedures for routine and emergency / disaster drug control as warranted o Perform other duties as required of a pharmacy consultant as set forth by applicable laws and regulations
