Specifications include, but are not limited to: The City of Chicago (“City”), acting through the Chicago Department of Public Health (CDPH), seeks invest in a continuum of care for women facing barriers to high quality breast cancer services with a focus on the priority populations of African American, Latina and low-income women. CDPH’s mission is to work with communities and partners to create an equitable, resilient, safe, and Healthy Chicago. Our efforts build toward our vision where everyone in Chicago thrives and achieves their optimal health and wellness. CDPH’s approach is guided by our community health improvement plan, Healthy Chicago 2025, that is focused on racial and health equity, especially eliminating Chicago's racial life expectancy gap. This RFP aligns with Healthy Chicago 2025’s theme to improving systems of care for populations most affected by inequities. Specifically, to increase access to comprehensive and culturally appropriate healthcare and social services among Black, Latina and low-income communities, with the goal to reduce inequities in breast cancer mortality among low income and women of color. In order to achieve these objectives is releasing two RFPs that seek to fund organizations that can address one or both of the following Community Breast Health Services RFPs, the current RFP is for category I only: Category 1: Breast Cancer Screening and Diagnostic Imaging (described in this RFP). Provide high quality mammography and diagnostic imaging services to uninsured or underinsured women from priority populations. Category 2: Patient Navigation. Provide evidence-based patient navigation to priority populations through the breast cancer continuum of care, addressing any barriers to timely diagnostic and treatment services and conducting community outreach to engage those out of care. The overall goal of these RFPs is to reduce inequities in breast cancer mortality through increasing access to high quality screening and diagnostic services and navigating women to early detection and treatment. Respondents who apply for both service categories must submit separate applications for both RFPs and propose a cohesive and coordinated program. This RFP is for Category 1: Breast Cancer Screening and Diagnostic Imaging. Provide high quality mammography and diagnostic imaging services to insured and underinsured women from priority populations.
