Specifications include, but are not limited to: Assist program staff and MDT in developing measures and data collection tools to assess the impact of the Implementation Plan, including each of the strategies outlined within the plan (attached). Identify and collect baseline data Develop evaluation and tracking tools Collect the identified data to measure performance on a quarterly basis Attend and co-facilitate (where applicable) at least monthly MDT meetings. Document implementation, and detail what changes are made along the way through program assessments. The evaluator will be responsible for providing timely, useful feedback to the MDT, including identification of implementation challenges and barriers. Monthly status report to the MDT Contribute to the quarterly grant report (performance measure progress)
