• Conduct therapy evaluations upon receipt of a written order from a resident’s physician. The evaluations should include a recommended plan of care, including duration and frequency of treatment. The evaluations should be completed within 48 hours and submitted for physician review and certification. Awarded Contractor will reply on written orders received from a resident’s physician to indicate medical necessity of therapy services. • Provide onsite evaluation and treatment services 6 days a week. • Provide onsite evaluation and treatment services to residents up to 6 times per week. • Proof of skills competencies, criminal background/fingerprint check and current information are to be available to LCNH upon request. • All therapists must be in possession of appropriate Illinois/national licensure/certification.
