The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform Fire Extinguishers Inspections Services at Captain James A. Lovell Federal Health Care Center in North Chicago, IL. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center (GLAC) Milwaukee, WI is soliciting businesses for Fire Extinguishers Inspections Services at Captain James A. Lovell Federal Health Care Center in North Chicago, IL. It is understood that all firms submitting a response to this Sources Sought will have an established business in the Fire Extinguisher Inspection / Testing industry with a minimum of three (3) years consecutive experience in performing these types of services in adherence with NFPA Standards. The firm must also be licensed or capable of obtaining the necessary licensing required to perform these services within the State of Wisconsin. The following NAICS Code has been identified for this requirement: 561621 - Security Systems Services (except Locksmiths) Interested firms shall provide an overview of their firm s capabilities and experience with projects similar in size and scope of this project. Capability statements must include: a. Company Experience in providing services, to include number of years and any examples of current contracts in place with government or commercial entities; b. Customer point of contact information for contracts listed in company experience along with a description of services provided under the contract (types of services provided, volume of trips per year, geographic service area covered, etc.); c. Number of technicians currently employed; d. Description of the ability to provide immediate emergency access and communication between customers and their personnel 24 hours a day, 7 days a week; e. Current hours of operation; f. Examples of Quality Control Programs and Procedures currently utilized by your firm; g. SAM UEI and DUNS number; h. Organization Name and Assumed Name/dba (if applicable); i. Organization Address; j. Point of Contact (including name, title, addresses, telephone number, fax number, and Email address); k. Business size under NAICS code 561621 (small business, other than small business, Woman-Owned, etc...); l. Socio-economic status (If SDVOSB or VOSB provide SDVOSB/VOSB CVE verification or VIP Profile); m. GSA contract number (if a GSA contract holder); n. Current state licensing possessed and a brief overview of that state s licensing requirements. If not currently licensed in Illinois, discuss any lead-times and processes your firm would take to obtain necessary licensing to perform these services in the State of Illinois. (All information shall be submitted in Word or PDF format, not to exceed 25 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 561621. If another NAICS code may be applicable, please identify the NAICS in your capability statement along with your corresponding business size and socio-economic status. Scope of Work: The National Fire Protection Association (NFPA) requires that portable fire extinguishers be inspected, tested, and maintained in accordance with NFPA 10. The VA adopts the latest edition of the NFPA Codes. This service is also required per The Joint Commission and OSHA. The Captain James A. Lovell Federal Health Care Center (FHCC), located at 3001 Green Bay Road, North Chicago IL 60064, has 650 portable fire extinguishers on its West Campus. The contractor shall provide all labor, supervision and all other resources required to deliver the requirements stated herein: All inspections will be conducted using the most recent NFPA guidelines. All third-party administrative filing fees are not included and will be passed on to the customer. All inspections will take place during normal business hours (7AM-3:30PM) Monday-Friday. Perform FIRE EXTINGUISHER inspections Monthly on up to (650) extinguishers. Perform annual FIRE EXTINGUISHER inspections on up to (650) extinguishers. Annual maintenance is to be conducted in September of each year following all requirements listed in the latest edition of NFPA 10. The annual maintenance is to be conducted by a technician certified in accordance with the latest edition of NFPA 10. The annual maintenance is to include a dusting/cleaning/wipe down of each unit and re-installation of tamper seals and contractor provided tags dated with the current year. This maintenance will be documented on mutually agreed upon tags (contractor furnished) provided on each extinguisher and will include the month/day/year of maintenance. The contractor will document the annual maintenance on the back of the tag in month of service and initial. The inspection will also be annotated on the inventory log provided by the VA verifying that the extinguisher received the required annual maintenance. Contractor shall perform internal inspections and/or hydrostatic testing as required. Contractor shall provide and install temporary fire extinguishers in place of those units being serviced. General Requirements All work is to be done between the hours of 7:00 AM and 4:00 PM, Monday through Friday unless otherwise specified. Contractor must possess and furnish written proof of a fire extinguisher manufacturer factory service certification and/or National Association of Fire Equipment Distributors Portable Fire Extinguisher Technician certification relevant to the type of work performed. Work dates shall be scheduled with the point of contact. Contractor will report to the VA Safety Office prior to starting any work to sign in and obtain a copy of the portable fire extinguisher inventory log. VA will furnish no tools, supplies or employees to assist with inspection and test work. Upon completion of all work, the contractor must provide a report indicating maintenance performed and pass or fail criteria, to include any corrective action taken for extinguishers required to be removed from service. This serialized list must identify compliance with the latest edition of NFPA 10.
