OVERVIEW AND PURPOSE: Packard Mental Health Center is seeking a vendor for asphalt repair services on campus roads and parking lots. Vendor will be responsible for pothole repair and painting/restriping roads and parking lots throughout campus located at 901 Southwind Rd., Springfield, Il 62703. SUPPLIES AND/OR SERVICES REQUIRED: Vendor will be responsible for repairing all potholes in roadways and parking lots. Vendor will provide all labor, materials, tools, equipment, and supplies necessary to complete the project. Vendor will remove top pavement overlay where needed up to 3 ½ inches and install new pavement overlay (hot-mix asphalt) up to 3 ½ inches. Asphalt will be graded towards drains to keep drains free of debris. Vendor will repaint and restripe letters, lines, and symbols throughout the campus. There are 365 parking spots at the Center (17 of those are handicapped parking spots), 10 crosswalks, painted curbs and 15-minute parking lines. Vendor will modify and paint handicap spots according to code and stripe walkways with hash marks. The Vendor shall report to the Facility Chief Engineer and/or designee any hazardous conditions and/or items in need of repair, etc. The Contract Monitor for this contract will be the Chief Engineer. Continued monitoring will be conducted from the start to completion of the project.
