The Illinois State Police (ISP) Division of Forensic Science is seeking a vendor or vendors to provide proficiency tests listed in the line items below. VENDOR IS REQUIRED TO HAVE ISO 17043 ACCREDITATION. Vendor must have available their accreditation certificate prior to purchase. Vendor(s) must be able to provide proficiency tests acceptable for the Illinois State Police's compliance with ISO 17025 requirements for accredited forensic science laboratories. The resulting contract with the awarded Bidder shall have an initial term of contract execution-June 30, 2027. Vendors are required to bid on all line items to be considered for award. Award will be based on lowest total cost on all lines items. SHIPPING CHARGES MUST BE INCLUDED IN THE UNIT PRICE. Vendor must be able to ship tests based on time frames given to them by ISP at the time of ordering. NOTE: Not all items will be shipped to the same address. Shipping is to multiple addresses. The addresses will be identified at time of purchase. List of addresses and delivery information can be found in attachments. The number of tests required by each laboratory will be determined at the time the order is placed. Each lab will be placing orders on an as needed basis and will provide the address at the time of ordering for shipping. The quantities listed in the bid are estimated and are not a guarantee for order. The quantities listed in bid buy line items are for the entire contract term. The agency will be purchasing the tests on an as needed basis. 161 EA DNA (1ST HALF) TEST--A FORENSIC PROFICIENCY TEST EACH CALENDAR YEAR TO ANALYZE SAMPLES FOR THE PRESENCE OF DNA; TEST MUST BE PROVIDED WITHIN ONE MONTH AFTER, BUT NOT PRIOR TO JANUARY OF EACH CALENDAR YEAR. 16 EA DNA (2ND HALF) TEST--A FORENSIC PROFICIENCY TEST EACH CALENDAR YEAR TO ANALYZE SAMPLES FOR THE PRESENCE OF DNA; TEST MUST BE PROVIDED WITHIN SIX MONTHS OF THE DNA 1ST HALF TEST. 158 EA FORENSIC BIOLOGY TEST--A FORENSIC PROFICIENCY TEST EACH CALENDAR YEAR TO SCREEN SAMPLES FOR BODY FLUIDS AND ANALYZE SAMPLES FOR THE DNA.
