Specifications include, but are not limited to: A. Data Collection and Entry 1. Establish a monitoring system to track timely collection of forms from completed vaccination clinics. 2. Prepare, collect, organize and scan forms gathered from clinics. 3. Review data from consent forms and vaccine administration records and extract pertinent information. 4. Process documents in accordance with established policies and procedures. 5. Enter data from forms into databases and spreadsheets. 6. Summarize data and compute averages, percentages, ranges, and other statistical measures. 7. Set up and maintain electronic and paper files and records of forms, reports, and logs. 8. Access and edit databases. B. Administrative Support 1. Coordinate printing and distribution of forms. 2. Create and prepare text and spreadsheet documents and reports relevant to immunization clinic operations. 3. Review information to ensure conformance with established policies and procedures. Notify supervisor of any discrepancies. 4. Coordinate and assemble material for mailing. 5. Prepare and distribute mass email communications.
