Specifications include, but are not limited to: DOH is seeking an organization to provide community Peer re-entry support services to Oahu jail and prison custodies (excluding inmates being paroled). Contractor shall provide peer support reach-in through community reentry services to Oahu jail and prison custodies (excluding inmates being paroled). 1. Develop a system with the Oahu jails and prisons to determine eligible clients 2. Develop an approval process to access custody information pertaining to reentry planning. 3. Forensic peer specialists meet with each qualified person according to the following: a. For prison custodies maxing out, at least 2 weeks before their scheduled release date in order to conduct a client assessment and determine custody participation and level of forensic peer support reentry services needed b. For jail custodies, upon the completion of jail intake and assessments, coordinate a time with PSD case managers to identify jail custodies who are homeless prior to intake or at high-risk of being homeless upon release from jail and schedule an interview at USH’s earliest availability c. Maintain a list of offenders accepting and declining forensic peer specialist reentry services and submit this list with the required quarterly reports to DOH.
