The ACSB client(s) will receive case management services either while living at the contracted clean and sober home/s or while in the community. The case management program will meet the client at the clean and sober home or in the community and provide an individualized approach to the client by addressing their basic needs while utilizing some motivational interviewing skills to assist them with being successful in their recovery. The case manager will meet with the client at the clean and sober home or in the community weekly to review their case plan and make an adjustment to the plan, if needed. Case management services have been budgeted through the Comprehensive Opioid, Stimulant, and Substance Use Site-Based Program (COSSUP) grant for approximately sixty (60) clients, two (2) hours per week, for up to twenty-four (24) weeks.
