Specifications include, but are not limited to: The purpose of the funding is to implement culturally tailored, population-wide interventions to address preventable risk behaviors related to tobacco use, poor nutrition and physical inactivity, among priority populations. The benefit must be to the whole state, and to reach and effect impactful outcomes for the greatest portion of the target populations, which will include Asian Americans and Native Hawaiian/Other Pacific Islanders. The target populations have been selected from a list of five priority populations identified by the CDC with the highest risk, or burden, of chronic disease. The request is issued for the purpose of including provider(s) in the federal grant application and if awarded, may result in a contract with the provider(s).
