The Hawaii State Department of Health (DOH), Family Health Services Division, Maternal and Child Health Branch (MCHB), Home Visiting Services Unit (HVSU) – referred to as “the State” or “HVSU” – is requesting proposals from qualified applicants that have the capacity and expertise to fulfill the roles and responsibilities of a PAT state office. As the representative for PAT programs in Hawaii, they will provide leadership through collaboration, communication and promotion activities. The HVSU supports a network of home visiting programs named the Your ‘Ohana Network (YON), of which most of the programs utilize the PAT home visiting model. The YON supports home visitors that provide pregnant women and families, particularly those considered at-risk, with necessary resources and skills to raise children who are physically, socially, and emotionally healthy, and ready to learn. Eligible families are referred to home visiting programs within the YON for voluntary enrollment in home visiting services. Services include but are not limited to: maternal and child health, child maltreatment prevention, school readiness, domestic violence, family economic self-sufficiency, and coordination and referrals for other community resources and supports. As the entity closest to those implementing PAT in Hawaii, the PAT state office serves as a prime and valued source of relevant information, resources, and training for PAT affiliates, providing critical guidance, technical assistance and support as affiliates comply with and build upon the essential requirements of the PAT home visiting model.
