This contract will be for a period of twelve (12) months starting from the award date and will provide medical transportation, round trip pick-up and drop-off of inmates from all Oahu correctional facilities to and from their scheduled medical appointments. I. Transport Capability: a. Vendors must provide appropriate transport vehicles, including those equipped to handle gurney-bound individuals. b. Transport must accommodate inmates weighing 400 lbs. or more, including the provision of adequate manpower to safely lift and transport such individuals. II. Fees and Charges: a. No cancellation fees shall be charged if notice of cancellation is provided within twenty-four (24) hours of the scheduled transportation. b. All fees and charges, including any additional or incidental costs such as waiting time, must be clearly disclosed and itemized in the proposal. No undisclosed or hidden fees will be permitted. III. Billing and Invoicing: a. Invoices shall be submitted monthly and shall reflect services provided within the preceding thirty (30) days. b. Each invoice shall include the date of service, name of the inmate transported, pick-up and drop-off locations, duration of service and itemized list of all charges associated with each transport.
