The CONTRACTOR shall, in a timely and satisfactory manner as determined by the STATE, procure all necessary materials and perform all services described in the SOW in strict accordance with the terms and conditions specified in this solicitation, which includes ensuring necessary permit(s) are in place before the start of construction. The STATE-assigned thirdparty PM shall submit documents to process the permit(s) with the City and County of Honolulu, Department of Planning & Permitting (“DPP”) office. The “Architectural Floor Plan,” for the construction of converting the powder room and laundry room to an accessible full bathroom in the Participant’s private residence on Oahu is attached hereto as Exhibit “A.” 1. PRE-CONSTRUCTION WALKTHROUGH This phase may begin after the contract execution and before obtaining the approved permit(s) from the DPP. The CONTRACTOR shall: A. Coordinate a walkthrough at the construction site to discuss the implementation plan and timeline with the PM before construction begins. B. Communicate with the PM and the family member designated by the PM regarding the timeline and status of the construction throughout the project. C. Document the condition of the interior and exterior of the home where the work shall be conducted, including photos of the construction area and the surrounding area. D. Review the documentation with the PM and the family and obtain their signatures confirming whether any damages were identified during the Pre-Construction Walkthrough. E. Provide a copy of the signed document to the PM by secure email, within two (2) business days of the Pre-Construction Walkthrough. F. Obtain the approved permits submitted by the PM from the DPP office within three (3) business days of notification from the PM that the permits are ready. G. Complete the steps above for the Pre-Construction Walkthrough within thirty (30) calendar days of the execution of the contract or upon approval of the permits from DPP, whichever is later.
