CCH services to be provided shall be described and shall include, but are not limited to the following: 1. A detailed description of the CCH model (e.g., Central Authority, Lead Agency, Federated) it plans to utilize and clearly lists its CCH proposed core functions. It shall be an approved evidence-based health promotion program by the Administration for Community Living. 2. A detailed written description on its means and method to implement the CCH services. 3. A detailed written description of its timeline to implement its CCH services. 4. Evidence that the program staffs properly utilize proven strategies of CCH services. 5. Use of best practices/evidence-based practices. Best practices/evidence-based practices are defined as a body of contemporaneous empirical research findings that produce the most efficacious outcomes for persons involved in domestic violence, has literature to support the practices, is supported by national consensus, has a system for implementing and maintaining program integrity, and conformance to ethical/professional standards. 6. State its ability and willingness to provide adequate personnel to deliver the CCH services. Provider needs to CLEARLY present in writing their proposed staffing ratio in the written response to this solicitation. 7. State its ability and willingness to follow all applicable Federal, State, and County laws and the rules and regulations of DOH /EOA. 8. State its ability and willingness to maintain financial, contractual records. 9. State its ability and willingness to provide periodic, and as requested, reports and evaluations to EOA. 10. State its ability and willingness to openly communicate with the EOA staff, governmental staff, members of the community, and all other stakeholders.
